This Release Form is the typical form we use here at Inside Out Student Ministries. This form should be filled out for each event your teen would like to participate in. I have an Electronic PDF version of our form available for your convenience. This way you can fill the Electronic PDF Release Form out once and print it for a fresh signature for each recurring event.
Why not use a yearly blanket Release Form?
Normally at the beginning of each year Inside Out attempts to get new blanket release forms filled out which maybe adequate for certain events but the safest thing to do is to renew the form each time. This enables you the ability to correct any minor and/or major changes in medical history, phone numbers, physicians or policy information.
PDF Electronic Release Form
The PDF Electronic Release Form can be filled out either electronically or by hand. However the final copy given to Inside Out Student Ministries for any event must have a parent or guardian signature and date indicating the acknowledgment of said person(s) release of all claims. This signature also holds Inside Out Student Ministries unaccountable for accidents that may happen causing sickness, injury and/or death.
